The tip of the appendix

The answer

There is a faecolith and the tip of the appendix.

The most important investigation would be to confirm that the recovered faecolith was responsible for the opacity seen on the PFA and CT, this is most easily accomplished by performing a further PFA.

The student might say

The image shows a brown object to the left that looks like a faecolith. To the right of this there is a structure that looks like the tip of the appendix.

The appendix tip should be sent for histological examination. Removal of the opacity seen on the original films would be best confirmed by repeating the PFA, although an alternative would be to X-ray the faecolith and compare it with the opacity seen originally.

Small Print

Ascaris is a worm and does look tube like, but this structure is the tip of the appendix. Remember the old name for the appendix is the vermiform appendix (worm like appendix).

A Meckel's divertiulum is larger.

A splenuculus is an accessory spleen, it appears spleen like.
