Ileal brake

Loss of the ileocaecal valve speeds intestinal transit as does resection of a part of the ileum.

Loss of the ileocaecal valve promotes colonisation of the small bowel with colonic flora. These flora may ferment carbohydrate to generate D-lactic acid. This D-lactic acid cannot be metabolised in humans and may build up to cause metabolic acidosis. This should be suspected if there is an increased anion gap and the patient developed encephalopathy.
