The patient receives a hand written letter of discharge for their GP, which includes dates of admission/discharge, admitting consultant, discharge diagnosis, procedures, all discharge medications, including those they were previously on and follow up arrangements. This is particularly important if a patient has died and needs to be discussed with senior team member before being sent.
