Record each days blood results, especially the important and complicated
cases. Results must be kept up to date daily.
- Record a summary of iv fluid therapy, especially blood products. Fluid
charts may not be kept after the patient has been discharged, and it may not
be clear later if a patient has been transfused if this is not recorded in the
clinical notes.
- Record date of diagnosis of wound infection and date of healing Record
the dates that sutures and drains were removed
- Record arrangements for outpatient follow up, including suture removal,
dressings Make sure you record drugs prescribed at the time of discharge from
hospital Record if a decision was made not to review the patient in the
outpatient clinic
Sign legibly ALL entries in the chart and print name underneath.
