Start with a bolus of 5,000 IU, and 1,000 IU/hr infusion after that. Check
coagulation 6 hours later.
- If APTT is
70 seconds, increase heparin by 100 IU/hr.
- If APTT is between 70-100 seconds, maintain the same dose.
- If APTT is between 100-120 seconds, decrease heparin by 100 IU/hr.
- If APTT is
120 seconds, stop for 2 hours,
recommence at 100 IU/hr lower than previous dose
- Repeat Coag 4-6 hours after changing heparin dose.
Heparin can-be reversed by Protamine Sulphate or infusion of fresh frozen
plasma (there is a risk of anaphylaxis when using protamine sulphate)
Only reverse heparin in consultation with Senior Team Member
