Check lists upon accepting transfers

• Obtain details of patient to be transferred - Name, DOB, address.

• Short history of how patient presented to the peripheral hospital

• Past history - medical and surgical background

• Current condition of patient - vitals and physical examination

• Latest investigative results - bloods - e.g. Hb. Radiological scans - USS, CT.

• Reason for transfer

• How fit patient is for transfer

• Who will be accompanying patient upon transfer - anaesthetist, intern, nurse

• Psychiatric patients should have a special nurse to accompany him/her and stay if necessary.

• The team requesting transfer should always accept the patient back when the emergency has been dealt with.

• Always record the name of the person requesting the transfer and the consultant in charge of patient.
