- Short history of how patient presented to the peripheral hospital
- Past history - medical and surgical background
- Current condition of patient - vitals and physical examination
- Latest investigative results - bloods - e.g. Hb. Radiological scans -
USS, CT.
- Reason for transfer
- How fit patient is for transfer
- Who will be accompanying patient upon transfer - anaesthetist, intern,
nurse
- Psychiatric patients should have a special nurse to accompany him/her
and stay if necessary.
- The team requesting transfer should always accept the patient back when
the emergency has been dealt with.
- Always record the name of the person requesting the transfer and the
consultant in charge of patient.
