Orientation

This shows two images taken at the time of laparoscopic cholecystectomy.

The upper part of the images corresponds to the front of the patient. The right part of the images corresponds to the patients midline. The left part of the images corresponds to the patients right hand side. The view is as seen from the laparoscope which has been introduced through the umbilicus and is looking up into the porta hepatis.

Description

In both images the surgeon is holding one instrument with the left hand and another instrument with the right hand. The instrument on the left is holding the gallbladder over Hartman's pouch. The instrument on the right is being used to dissect the cystic duct where it comes off the gallbladder.

In the image on the left the dissection has just commenced, the peritoneum over the cystic duct has been opened.

In the image to the right, the dissection is further advanced, the cystic duct has been identified and the intstrument is being carefully passed behind the cystic duct.
